COMMUNITY CASE MANAGEMENT

The Community Case Management opened in 1999 to fill a gap in services in Washington Heights and Inwood by providing a full range of case management services to anyone in the community who is over 60 years of age. Staff of Community Case Management make home assessments and refer seniors to available services or provide services directly. In addition to overall care management, our services include daily money management for seniors who are having difficulty managing their incomes and chore services (shopping, laundry, light housekeeping and escorting), for people who do not fall within the regular home care guidelines. Fees are based on a sliding scale.

- Assistance with services and entitlements such homecare, Medicaid/Medicare, food stamps
