Coordinating care for Ms. D

Ms. D is 79-year-old immigrant from Puerto Rico who is divorced and lives alone in an apartment located in a public housing building in Central Harlem.  She used to work for the Bronx Botanical Garden.  She has been coming to Riverstone’s senior center for the past years.  She volunteers and participates in many of the senior center activities.

As Ms. D has aged she has needed additional support

Ms. D meets with her social worker regularly and receives assistance on a monthly basis for  paying her rent and for submitting her surplus income to Medicaid. She  requested and was approved as a client in Riverstone’s Health Home case management program (through a partnership with NewYork-Presbyterian Hospital). Now Ms. D not only receives bill payment assistance but also receives comprehensive social service assistance including coordination of care with her health providers.
